Controversy and uncertainty is the starting point of any investigation. It is the clinical uncertainty and the controversy around that uncertainty which irritates some doctors enough to do something about it. If we do not recognise there is uncertainty, and if we are not uncomfortable with such, it is likely to lead to acceptance of ignorance. The fundamental starting point has to be the uncertainty and resulting discomfort. This could mean that patients then run the risk of being managed less well, because doctors are uncertain about best management. Having just completed the three yearly cycle with consensus, we are currently looking around for the topics not yet resolved. To move this forward, it is often helpful to debate both sides of a story. But however it is done, we must investigate the clinical uncertainty to be able ultimately to achieve a better future for our patients. This is the opposite of complacency. As well as the value of the topic, it can also be most enjoyable as the debates bring both the best and worst out of medical experts. I have thoroughly enjoyed editing the book and look forward very much to the presentation in real life.
